How to Prepare for Discharge and Manage New Medications
Leaving hospital can feel like a relief, but it is also important to understand any new medicines you have been given before you go home.
At The Pharmacy Shop, we dispense discharge medicines for patients coming from Medical Same Day Emergency Care (MSDEC), Surgical Same Day Emergency Care (SDEC) and Paediatric Same Day Emergency Care (PSDEC). Our team is here to help make the discharge process smoother by supplying your medicines and offering clear advice on how to take them safely.
Before leaving hospital, check that you understand:
- What each medicine is for
- How and when to take it
- Whether it should be taken with food or at a certain time of day
- Any important side effects to look out for
- Whether any of your usual medicines have changed or stopped
- What to do if you miss a dose
It can also help to keep your discharge paperwork and medicine labels together, especially if you need to speak to your GP, hospital team or pharmacist after leaving hospital.
If you are unsure about your new medication, please ask before you leave. Our pharmacy team can help explain your medicines clearly so you feel more confident managing them at home.
